Getting your hospital off the ground

Step
In short Laying down beds, a diagnosis desk, and at least one nurse gives you a working hospital.

When you first join, your plot is empty and no patients will arrive until you have the basics in place.

  1. Place beds every bed you put down raises the number of patients the emergency system can deliver.
  2. Add a diagnosis desk patients need to be checked before treatment can begin, and the desk is where that happens.
  3. Hire your first nurse nurses handle the front desk, diagnosis, and basic treatments, and the first one is always free.
  4. Research the ambulance open the Manage panel, go to the Research tab, and unlock Emergency Delivery so patients are brought to you automatically.
  5. Place a heal machine match it to the disease you have researched; without a machine the staff cannot treat the patient.
  6. Add a soda machine and waiting bench these count as bonus items in the delivery formula and push up the number of patients each emergency drops off.

How emergency delivery works

Summary
In short The more beds, desks, heal machines, and bonus furniture you have, the more patients each emergency run delivers.

After you unlock Emergency Delivery through research, ambulances arrive on a 30-second timer. Which tier fires depends on your hospital size, and the exact number of patients dropped off is calculated from everything listed below.

  • Small tier supports up to 10 patients, 50% chance to spawn, arrives in an Emergency Car
  • Medium tier supports 10–40 patients, 75% chance to spawn, arrives in an Emergency Ambulance
  • Big tier supports 40–80 patients, 100% chance to spawn, arrives in an Emergency Bus
  • Delivery formula (Beds × 2) + (Desks × 0.95) + (Heal Machines × 3.5) + (Bonus Items × 1.75) ÷ 8.2
  • Bonus items counted Soda Machine (×0.5), Waiting Bench (×0.75), Trash Can (×0.5)
  • Hard cap the result is still capped by your actual bed count

Diseases, research, and what each one needs

Table
In short Unlock a disease's research first, then have the right machine and worker ready before that patient type can enter your hospital.

Each disease requires its own research to be unlocked, a specific healing machine, and either a nurse, doctor, or surgeon. Rarer diseases pay more cash per cure. The Earning Boost gamepass raises every reward to 1.5× the base amount.

Hiring the right workers

Table
In short Match your worker types to the diseases you have researched, since using a cheaper nurse where a surgeon is required will leave patients untreated.

You can employ up to 50 workers total. Each type charges a scaling cost based on how many of that type you already have.

Nursecheapest option ($500 × number of nurses), handles reception, beds, diagnosis, and common diseases
Doctormid-cost ($1,500 × number of doctors), diagnoses and treats more advanced diseases, cannot work reception or monitor beds
Surgeonmost expensive ($5,000 × number of surgeons), operates every healing machine and can cure every disease in the game
Janitorlow cost ($400 × number of janitors), keeps the hospital clean, no medical duties

Vehicles for patient pickup

Table
In short Emergency vehicles bring patients to your hospital; speed cars and others are for general travel. Capacity is how many patients the vehicle can carry in one trip.

Common questions

FAQ

Why aren't patients with a certain disease showing up?

You need to unlock that disease's research before those patients will appear. Open the Manage panel, go to Research, and purchase the relevant entry. After that, make sure you also have the correct healing machine and the right worker type placed in your hospital.

How do I get more patients per emergency delivery?

The delivery formula weighs beds most heavily, so adding more beds raises the number. Heal machines and bonus furniture like soda machines, waiting benches, and trash cans also contribute to the total. The final number is still capped by your bed count.

Do I need a surgeon for every disease?

No. Common diseases like Noobvirus and Emotion Overflow only need a nurse. The Sus disease accepts either a doctor or a surgeon. Check the disease table to match the right worker before unlocking a new research.
